Learning from Disasters One Day Program – Mudgee

Thanks to all those participants from Peabody Energy (Wilpinjong Mine) and Yancoal (Moolarben Open Cut Mine) that attended the One Day Program of Learning from Disasters, facilitated by Leadership & Performance Partners in Mudgee on Thursday 28th November 2019.

This program, developed by the NSW Resources Regulator incorporates the findings from investigations and independent reviews of mining incidents that involved fatalities and serious injury, or where the circumstances presented a significant risk of injury or death to workers or the wider community to ensure mine workers learn from past mining disasters.

Practising certificate holders (excluding holders of practising certificates restricted to a specific mine) must, as a minimum, complete either the Regulator’s Learning from disasters one-day program; or a minimum seven hours of other formal learning on disasters for the ‘General WHS’ area of competence during the five year period.

The program has the following learning objectives:

  • Recognise recurrent patterns of failure linked to significant work health and safety incidents.
  • Understand how identifying and managing a principal hazard links to reducing risk.
  • Recognise the causal role that human and organisational factors and critical factors play in work health and safety incidents.
  • Build an understanding of roles, responsibilities and mechanisms that foster a safety culture.
  • Identify contemporary risk factors and vulnerabilities in your safety management systems.
  • Develop risk control options and strategies that best manage work, health and safety risks.

During the day, the participants reviewed past disasters and using Professor Michael Quinlan’s theory in relation to 10 Pathways identified factors that led to the disasters occurring.  Consideration was given to principal hazards in their own mining environments and the actions that could be undertaken to prevent an incident from occurring, using the lessons of history to identify those factors.
